Position Summary
Provides quality care to patients in their homes under the direction of a physician. The Senior Home Health RN functions as a
consultant in the nursing assessment of patients who are managed by other nurses or disciplines in the department and coordinates
patient care activities with other members of the health care team. Position includes responsibility for the assessment,
treatment, and care of patients of all ages including adults, pediatrics and geriatric. The treatment of patients is individualized and
reflects an understanding of the developmental needs and range of treatment requirements. Senior Home Health RN is responsible for
monitoring medical, physical, psychiatric and wound care needs of assigned patients in Home Health.

Position Summary
Provides quality care to patients in their homes under the direction of a physician. The Senior Home Health RN functions as a

patient care activities with other members of the health care team. Position includes responsibility for the assessment,
treatment, and care of patients of all ages including adults, pediatrics and geriatric. The treatment of patients is individualized and
reflects an understanding of the developmental needs and range of treatment requirements. Senior Home Health RN is responsible for
monitoring medical, physical, psychiatric and wound care needs of assigned patients in Home Health.
Position Specific Responsibilities
- Maintains homecare standards of nursing care and provides care in accordance with policies and procedures. Possesses current clinical skills and demonstrates broad scope of clinical knowledge.
- Arrange for nursing visit if needed. Provide follow-up communication to other Home Health staff regarding patient/family needs.
- Coordinates and evaluates patient care that includes interdisciplinary services, treatments, diagnostics and preventative procedures with other members of the multi-disciplinary team and with physicians.
- Anticipate disease progression and implications for symptom management.
- Inform patient and family of current and anticipated changes in patient condition as appropriate.
- Participates in orientation of team members to the agency.
- Maintains agency productivity standards.
- Responsible for understanding patients’ insurance benefits and managing care in a cost effective manner.
- Evaluate and document patient and family response to interventions, revise the Plan of Care as appropriate.
- Document RN assessments, interventions, and communications accurately and timely according to established standards.
- Communicate changes in Plan of Care as appropriate or requested.
- Contribute to overall team function through effective time management and communication; utilize critical clinical thinking and problem-solving skills to address team issues.
- Responsible for maintaining awareness of professional Scope of Practice and Conditions of Participation.
- Adhere to ministry policies and procedures, and applicable regulations.
- Participate in continuous quality improvement activities, identify related issues and implement recommended changes as appropriate.
- Provide consultation for speciality patients as well as to nurses for speciality patients.
- Reports acute medical problems, condition changes including changes in social condition and environment to attending physician.
- Demonstrates competency in skills and knowledge pertinent to the practice of a registered nurse. Performs procedures and treatments according to accepted department standards.
- Attends mandatory meetings. Participates in activities which maintain/enhance professional expertise and clinical skills (continuing education courses, workshops, in-service presentations, professional group involvement, self-study, literature review, mentoring other staff). Shares learning with peers.
- Adjusts daily schedule according to workload.
- Supports teamwork within the department.
- Provides written instruction to and supervises the assigned Home Health Aide.
- Continually evaluates the quality of one’s nursing practice by identifying areas of strength and limitation, accepting responsibility for competent actions and judgment, and understanding legal consequences of nursing actions.
- Complies with all applicable Federal, State, and local laws, regulations, and requirements as well as facility specific policies and procedures. In addition, all employees who have contact with patients are expected to promote the Patients Bill of Rights and Responsibilities and understand the basic procedures for receiving and documenting grievances in order to initiate the appropriate process for participant concerns.
- Maintain confidentiality of all patient demographics, medical and financial information at all times. Maintain confidentiality of all company and patient information. Ensure proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations.
- Adheres to all policies and procedures including regulatory requirements with regard to infection control, practicing universal precautions, sanitation, and safety and disaster preparedness.
- Performs other duties as assigned.
- Required - Bachelor's Degree Nursing
- Required - 3 years Nursing experience.
- Preferred - Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology.
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
- Language Skills: Excellent communication and organizational skills; able to effectively communicate complex information to diverse audiences.
- Computer Skills: Proficient in using computers and Microsoft Office Suite programs required; knowledge of Electronic Health Record systems preferred.
- Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
- Other Skills and Abilities: Demonstrated skills in clinical nursing assessment and treatment skills required; knowledge of hospice and palliative care practices preferred.
- Other Qualifications: Knowledge of the Medicare Conditions of Participation and other regulatory guidelines.
- Other Skills and Abilities: Knowledge and alignment with Providence philosophy and standards of care; able to work in a way that is consistent with/supportive of Ministry’s mission, philosophy, goals and objectives.
- Other Skills and Abilities: Demonstrates competency by maintaining positive, collaborative, and constructive interpersonal relationships; understands and practices the principles of effective teamwork.
- Other Skills and Abilities: Demonstrated ability to effectively work with families in crisis while maintaining professional boundaries and personal self-care strategies for dealing with stress.
- Other Skills and Abilities: Proven ability to build effective and professional relationships with a wide variety of people at all levels of the organization, both within and outside of the Providence system.
- Other Skills and Abilities: Knowledge of and sensitivity to a wide variety of cultural and value systems.
- Other Skills and Abilities: Able to prioritize work assignments and meet productivity and quality standards; work with minimum supervisory guidance and exercise independent judgment within the scope of practice; is willing to assume additional responsibility and learn new procedures.
- Required upon hire: Washington Registered Nurse License (Vendor Managed)
- Required within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed)
- Required within 30 days of hire: National Provider Identifier (NPI) number
- Required upon hire: Certified Wound Ostomy Continence Nurse (CWOCN) Or
- Required upon hire: Certified Wound Care Nurse (CWCN) Or
- Required upon hire: Certified Ostomy Care Nurse (COCN) Or
- Required upon hire: Certified Continence Care Nurse (CCCN) Or
- Required upon hire: Educator Diabetes Or
