Position Summary

The Triage RN is responsible for ensuring the smooth delivery of services by coordinating communications, services and orders between patients, providers, and clinicians to meet patient/family needs. The Triage RN functions independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs.

Position Specific Responsibilities

  • Perform triage activities such as receiving incoming phone calls from patient provider, providers' staff, patients and families, assessing patient needs, providing immediate troubleshooting/problem-solving over the phone.
  • Provide nursing advice and triage using sound nursing assessment skills and following approved protocols.
  • Document all patient encounters in the electronic medical record and other platforms as required, accurately and timely.
  • Contact providers to obtain verbal orders and process appropriately for treatment, labs, wound care, extensions of service, addition/removal of services, and more.
  • Arrange for in-person or virtual nursing visits as indicated and provide follow up communication to other staff regarding situation and needs.
  • Maintain relationships that promote optimal patient outcomes.
  • Communicate in an efficient and positive manner that fosters productive relationships and maintains high quality care.
  • Collaborate with community and bedside healthcare providers to ease transitions in care and set an individualized care plans that will enable the patient to be successful in managing their acute or chronic conditions.
  • Attends training opportunities and maintains technological competence.
  • Follow established workflows, policies, and procedures.

Education And/or Experience

  • Required - Graduate of an accredited/approved school of Nursing
  • Required - RN experience in an acute care hospital strongly preferred unless hired as an intern or fellow.
  • Preferred - Recent clinical experience in any of the following suggested areas; home health, tele health, medical-surgical, critical care, emergency, pediatric or maternal-child health nursing.

Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Language Skills: Excellent communication and organizational skills; able to effectively communicate complex information to diverse audiences.
  • Computer Skills: Proficient in using computers and Microsoft Office Suite programs required; knowledge of Electronic Health Record systems preferred.
  • Other Skills and Abilities:
    • Ability to use several software programs simultaneously.
    • Ability to accurately assess and triage patients using only audio and visual tools.
    • Ability to adapt to a rapidly changing healthcare environment.
    • Demonstrate competency and knowledge in utilization of specific department equipment and various software programs.
    • Excellent verbal and interpersonal skills as demonstrated by the ability to communicate clearly and efficiently.
    • Excellent listening skills as demonstrated by the ability to listen to understand multiple points of view.
    • Excellent phone skills including the ability to transfer and conference phone calls.
    • Ability to make decisions independently utilizing professional judgment.
    • Working knowledge of the principles of professional nursing.
    • Knowledge of commonly used medication and their effects.
    • Considerable knowledge of the behavior and needs of patients.
    • Knowledge of medical terminology and equipment.
    • Ability to evaluate and report changes in the physical and mental conditions of patients.
    • Ability to interpret and apply medical orders.
    • Ability to work with and advocate for a wide range of patients of varied socioeconomic and ethnic backgrounds.
    • Must be organized and self-motivated.
    • Driving may be necessary as part of this roll. Caregivers are required to comply with all state laws and requirements for driving.
Certifications, Licenses, and Registrations

  • Required upon hire: Registered Nurse License (Vendor Managed)
  • Required upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all sate laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.

  • Required upon hire: National Provider BLS - American Heart Association (Vendor Managed)

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $34.38 - $56.31

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
