St. Luke's Health System is seeking a Registered Nurse (RN) Home Health Case Manager, Acute Care Case Management for a nursing job in McCall, Idaho.

St. Luke’s home health team of skilled professionals provides comprehensive health care in the warmth and comfort of the patient’s home.  From newborn to seniors, we offer the latest home health techniques and technology to people at all ages and stages of life.  Dedicated to the continuum of care as patients transition from inpatient to home to outpatient settings for care.  Our home-based services team (NP, RN, MA, MSW) also assists with oversighting transitions.  Close working relationship with St. Luke’s Hospice team.

The Home Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care from admission to discharge for an assigned case load.  This is a direct care position providing in home care that requires traveling within McCall, ID and it's surrounding areas.

What you can expect from this role

  • Full Time Days (Mon-Fri from 8:30am-5:00pm) with rotating evening call and weekends
  • Manageable case-loads (Daily visit requirements below state and national average)
  • Pay for drive time
  • Mileage reimbursement (Geographical Area:  McCall team serves the following counties in Idaho:  Adams, Idaho, Valley, and Washington)
  • Applicable Differential/On-call Pay
  • Extra incentive for cost-of-living in McCall
  • Cross-training opportunities in hospice
  • Sign-on bonus plus relocation assistance available as applicable

Qualifications

  • Education: ASN or BSN Degree
  • Experience: 2 years of RN experience; Hospice or Acute Care experience helpful
  • Licenses/Certifications: Current RN License in the State of Practice; Current Basic Life Support (BLS) Certification
  • Valid Driver's License and proof of Auto Insurance
